E-1.  The committee recommends and I so move that approval be given to amend the 2012-2013 Out-of-District Atypical Student Placement List in accordance with the information in the hands of each board member.

E-2.    The committee recommends and I so move that approval be given to amend the list of students removed from the rolls [two (2)  students exited for the period October 1 through October 31, 2012; a total of  eleven (11) students exited for the 2012-2013 school year] who are not domiciled in this school district in accordance with the information in the hands of each board member.

E-3.    The committee recommends and I so move that the board approve Nonpublic No Child Left Behind (NCLB) Title I Agreement with Union County Educational Services Commission in accordance with the information in the hands of each board member.

E-4.    The committee recommends and I so move that the board approve NJQSAC District Performance Review for 2012-2013 school year in accordance with the information in the hands of each board member.

E-5.    The committee recommends and I so move that the board approve NJQSAC District Statement of Assurance for the 2012-2013 school year in accordance with the information in the hands of each board member.

J.        Fiscal and Planning Committee Resolutions:

 
F-1.    The Committee recommends and I so move that the Treasurer’s Report dated October 31, 2012 be accepted.

F-2.    The Committee recommends and I so move that the Secretary’s Report dated October 31, 2012  be accepted.

F-3.    Pursuant to N.J.A.C. 6:20-2.13(d), I certify that as of October 31st, 2012 no budgetary line item account has obligations and payments (contractual orders) which in total exceed the amount appropriated by the district board of education pursuant to N.J.S.A. 18A:22-8 and 18:22-8.1.

Pursuant to N.J.A.C. 6:20-2.13(e), we certify that as of October 31st 2012 after review of the secretary’s monthly financial report (appropriate section) and upon consultation with the appropriate district officials that to the best of our knowledge no major account or fund has been over-expended in violation of N.J.A.C. 6:20-2.13(b) and that sufficient funds are available to meet the district’s financial obligations for the remainder of the fiscal year. This certification is solely based on the information provided by the School Business Administrator and Board Secretary and is assumed by the board to be correct.

  

 F-4.    The committee recommends and I so move that the Board approve line item transfers in accordance with the copy in the hands of each board member.

 F-5.    The committee recommends and I so move that the Comprehensive Annual Financial Report (CAFR) and the Auditor’s Management Report be accepted in accordance with the copy in the hands of each board member and on file in the office of the Secretary.

F-6.    The committee recommends and I so move that the board approve contracts and/or purchase orders pursuant to the requirements of N.J.S.A. 18A:18A-5 and 18A:18A-10(a) and in accordance with the information in the hands of each board member.

F-7.    The committee recommends and I so move that the Board approve following list of contracts and/or purchases pursuant to the requirements of N.J.S.A. 18A:18A-5(21):  (a) IEPlanners Inc. - $3,422 – Volleyball team fundraiser – Union High School Athletics; (b) Scholastic Book Fairs - $2,217.17 – book fair purchases – Kawameeh Middle School; (c) Forest Lodge, LLC - $1,600 – 1st deposit for 8th grade trip on June 17, 2013 – Kawameeh Middle School; (d) Forest Lodge, LLC - $1,600 – 2nd deposit for 8th grade trip on June 17, 2013

F-8.    The committee recommends and I so move that the Board approve districtwide travel and related expenses pursuant to the requirements of N.J.S.A. 18A:11-12, N.J.A.C. 6A:23A-7 and Board Policy File Code 6471 and in accordance with the information in the hands of each board member.

F-9.    The committee recommends and I so move that the Board pre-approve districtwide student field trip destinations and purposes pursuant to N.J.A.C. 6A:23A-5.8 in accordance with the information in the hands of each board member.                       

F-10. The committee recommends and I so move that the Board accept the following donations: (a) Modell’s, Inc. – $108.07 for use by Burnet Middle School PBSIS Committee; (b) Target - $243.48 for use by Burnet Middle School; (c) Target - $553.54 for use by Washington Elementary School; (d) Union Taxi - $100 for use by Burnet Middle School; (e) Target - $474.85 for use by Connecticut Farms Elementary School; (f) Target - $54.34 for use by Hamilton Elementary School; (g) Target - $247.74 for use by Kawameeh Middle School

F-11. The committee recommends and I so move that the board approve renewal of Reimbursement Services Agreement with AFLAC in accordance with the information in the hands of each board member.     

J.       Operations Committee Resolutions:

O-1.   The committee recommends and I so move that pursuant to N.J.A.C. 6A:17-11.2 the board approve the following bus emergency evacuation and security drill reports in accordance with the information in the hands of each board member.

O-2.   The committee recommends and I so move that the board approve discard of textbooks in accordance with the information in the hands of each board member.

O-3.   The committee recommends and I so move that the board affirm the Superintendent’s resolution of Harassment, Intimidation and Bullying (HIB) conclusions for the period October 8th through November 12th 2012.   

O-4.   The committee recommends and I so move that the board approve waiver of rental fees for use of Battle Hill gym every Thursday from January through March 2013 from 7 to 9 p.m. by Crossroads Christian Fellowship Church, 2815 Morris Avenue, Union, New Jersey for their upcoming volleyball/basketball program. The organization will be responsible for utility costs as well as any custodial fees that apply. 

O-5. The committee recommends and I so move that the board approve Annual Review and Revisions of Uniform Memorandum of Agreement between Education and Law Enforcement Officials in accordance with the information in the hands of each board member.

O-6.   The committee recommends and I so move that the board accept proposal from Potter Architects, 410 Colonial Avenue, Union, New Jersey (Proposal #2012105) for Franklin Elementary School Intercom, Clock and PA/Sound System Upgrades in an amount not to exceed $13,800 in accordance with the information in the hands of each board member.

 

O-7.   The committee recommends and I so move that the board approve use of Union High School Main Gym on Sunday, December 16, 2012 from 8 a.m. to 4 p.m. by Township of Union Police Department PBA Local #69 to sort and pack up toys which were collected during the Santa In Blue Toy Drive for distribution.

                                    

K.      Personnel Committee 

P-1.    The Superintendent recommends, the committee concurs and I so move that Personnel Actions be approved in accordance with the information in the hands of each board member.

P-2.    The Superintendent recommends, the committee concurs and I so move that the Board approve the November 2012 Substitute/Home Instruction List, Substitute Custodians, Substitute Cafeteria/ Playground Aides; Classroom Assistants and Secretaries and Substitute Bus Drivers and Bus Aides and Substitute After School Monitors  in accordance with the information in the hands of each board member.

P-3.    The Superintendent recommends, the committee concurs and I so move that the board accept letters of resignation from the following staff:  (a)  Laura F. Adams, long term substitute, effective December 21, 2012; (b) Anthony W. Davis, SFC,USA,RET, Army Instructor at Union High School, effective August 1, 2013 (retirement)

P-4.    The Superintendent recommends, the committee concurs and I so move that the board approve leaves for the following staff: (a) Linda Steeb, math teacher at Kawameeh Middle School, unpaid family leave for the period October 25th, 2012 through January 25th, 2013; (b) Noel Cruz, part-time bus driver unpaid medical leave for the period November 1st, 2012 through January 1st, 2013; (c) Lisa Cassano, pre-school disabled teacher at Battle Hill School maternity and unpaid family leave for the period February 1st through June 16th, 2013; (d) Shannon Durak, physical education teacher at Burnet Middle School, extended child rearing leave for the period December 3, 2012 through January 1, 2013; (e) Megan Diaz, teacher at Livingston School, maternity and extended family leave for the period January 14 through May 31, 2013 (f) Christine Veglia, teacher at Connecticut Farms School, unpaid family leave for the period October 29th, 2012 through January 3rd, 2013; (g) Christine Drivas, A.A.P. teacher at Franklin and Livingston Elementary Schools maternity and unpaid family leave for the period December 22, 2012 through May 6, 2013 

P-5.    The Superintendent recommends, the committee concurs and I so move that the board amend list of Scoreboard Operators for wrestling matches to include Russ Cannavo at a rate of $60 for one game; $80 for two games and $125 for three games in accordance with the information in the hands of each board member.

P-6.    The Superintendent recommends, the committee concurs and I so move that the board approve security monitors for Union High School Production of “Xanadu” for performances on November 29 and 30, December 1 and 2, 2012 as follows: Helene Lateiner; Ron Lateiner; Valerie Desamours; Dee Stewart; Gayle Sumner; Mark D’Aloia; Patricia Frazier; Carlos Esquivel; Joe Florio; Tony Manderichio and Tony Stewart. There will be four monitors working each performance at a rate of $22.22 per hour for four hours or not to exceed $88.88 per person. The cost for each performance will not exceed $355.52 and the total cost for all four performances will not exceed $1,422.08.

P-7.    The Superintendent recommends, the committee concurs and I so move that the board approve Bergen County Special Services, 327 East Ridgewood Avenue, Paramus, NJ to provide Related Services for an out of district student at Millburn Regional Day School at a rate of $62 per 30 minutes, not to exceed $2480 for the 2012-2013 school year in accordance with the information in the hands of each board member. (Acct No. 11-000-216-320-01-19)

P-8.    The Superintendent recommends, the committee concurs and I so move that the board approve Mountain Lakes Board of Education, Lake Drive School, 10 Lake Drive, Mountain Lakes, NJ to provide interpreting services for extracurricular activities for a district student who attends the Lake Drive Program in an amount not to exceed $8,000 for the 2012-2013 school year in accordance with the information in the hands of each board member. (These services are in accordance with student’s IEP and will be charged to Acct No. 11-000-216-320-01-19)

P-9.    The Superintendent recommends, the committee concurs and I so move that the board approve Joanne Timpano, district occupational therapist, make a presentation regarding implementation of assistive technology at a rate of $40 per hour not to exceed $400 for the 2012-2013 school year. (Acct # 11-000-223-110-01-19)

P-10. The Superintendent recommends, the committee concurs and I so move that the board approve Daniel Hrdina, district teacher, to present at a parent workshop at a rate of $40 per hour for two hours or not to exceed $80 for the 2012-2013 school year (Acct # 11-000-223-110-01-19)

P-11. The Superintendent recommends, the committee concurs and I so move that the board approve Dalila DaTorre, district speech therapist to present at a parent workshop at a rate of $40 per hour for two hours or not to exceed $80 for the 2012-2013 school year (Acct #11-000-223-110-01-19)

P-12. The Superintendent recommends, the committee concurs and I so move that the board approve Daniel Seugling, district school psychologist, to present at a parent workshop at a rate of $40 per hour for two hours or not to exceed $80 for the 2012-2013 school year (Acct # 11-000-223-110-01-19)

P-13. The Superintendent recommends, the committee concurs and I so move that the board approve George McCloskey, Ph.D. to deliver a three hour workshop on Learning Disabilities to Child Study Team staff at a rate not to exceed $450 for the 2012-2013 school year in accordance with the information in the hands of each board member (Acct #11-000-219-320-02-19)

P-14. The Superintendent recommends, the committee concurs and I so move that the board approve dismissal of employee #10559 effective immediately. Employee will be paid for sixty (60) days as per contract that the Union Township Education Association (U.T.E.A.) has with the Board of Education.

P-15. The Superintendent recommends, the committee concurs and I so move that the board approve Salary Class Changes for teachers and teacher assistant stipends for the 2012-2013 school year in accordance with the information in the hands of each board member.

  
L.            Legal Committee
 

L-1.    The committee recommends and I so move that the board approve Settlement Agreement with Legacy Management Company in accordance with the information in the hands of each board member.

 
M.           Approval of Bills
