E-1. The committee recommends and I so move that approval be given to amend the 2014-2015 Out-of-District Atypical Student Placement List from out-of-district placement, in accordance with the information in the hands of each Board member.

The committee recommends and I so move that approval be given to amend the list of students removed from the rolls [zero (0) students exited from April 1st through April 30, 2015 or a total of thirty-four (34) students for the 2014-2015 school year] who are not domiciled in this school district and five (5) students who will be allowed to complete the 2014-2015 school year in accordance with the information in the hands of each Board member.

P-7. The Superintendent recommends, the committee concurs and I so move that the board reappoint Debra Cyburt as Treasurer of School Moneys for the 2015-2016 school year at an annual salary of $4,500.
P-8. The Superintendent recommends and I move that the board reappoint Manuel E. Vieira, as Affirmative Action Officer for awarding contracts for the 2015-2016 school year under P.L. 1975, c.127, at no cost to the District.
P-9. The Superintendent recommends, the committee concurs and I so move the board appoint James J. Damato, as Custodian of School Records for the 2015-2016 school year at no cost to the District.
P-10. The Superintendent recommends, the committee concurs and I so move that the Board tentatively approve Employment Contract of Manuel E. Vieira, business Administrator for the 2015-2016 school year in accordance with the information in the hands of each Board member. This contract has not been approved by the Executive County Superintendent.
P-11. The Superintendent recommends, the committee concurs and I so move that the Board tentatively approve Employment Contract of Noreen Lishak, Assistant Superintendent for the 2015-2016 school year in accordance with the information in the hands of each Board member. This contract has not been approved by the Executive County Superintendent.

P-13. The Superintendent recommends, the committee concurs and I so move that the Board approve residency investigators to work during Summer, 2015 at a rate of $20 per hour as follows: (a) Maria Schmidtberg – not to exceed 70 hours for July and August or $1,400, (b) Thomas Sheridan – not to exceed 50 hours for July and August for $1,000, (c) Anthony Martino – not to exceed 50 hours for July and August or $1,000; (d) John Matos – not to exceed 50 hours for July and August for $1,000; (e) Al Tutela - not to exceed 50 hours for July and August for $1,000; (f) Rich Catino - not to exceed 50 hours for July and August for $1,000; (g) Dan Hodge - not to exceed 50 hours for July and August for $1,000; and (h) Gary Abraham - not to exceed 50 hours for July and August for $1,000, in accordance with the information in the hands of each Board member.
P-14. The Superintendent recommends, the committee concurs and I so move that the Board approve Curriculum Writing Teams for Woods I, II, III and IV (Grades 9-12) at a rate of $20 per hour not to exceed 15 hours per guide or a total of $1,200.00 (for 4 guides), in accordance with the information in the hands of each Board member.
P-15. The Superintendent recommends, the committee concurs and I so move that the Board approve Morris Union Jointure Commission, 340 Central Avenue, New Providence, New Jersey, to provide: (a) physical therapy - $225/hr., (b) occupational therapy - $200/hr., (c) speech therapy - $240/hr., (d) personal aides for ESY - $10,222 each and personal aides 10 months - $68,150 each, in accordance with the information in the hands of each Board member (Acct. #11-000-216-320-01-19).
P-16. The Superintendent recommends, the committee concurs and I so move that the Board approve Helene Sarnell to provide speech-language services at a rate of $85/hr (not to exceed $8,500 for the 2014-2015 school year), in accordance with the information in the hands of each Board member.

The Superintendent recommends, the committee concurs and I so move that the Board approve Inlingua to provide translation and interpretation for the 2015-2016 school year (not to exceed $3,000), in accordance with the information in the hands of each Board member (Acct. #11-000-216-320-01-19).

The Superintendent recommends, the committee concurs and I so move that the Board approve Caldwell Pediatric Therapy Center (1129 Bloomfield Avenue, Suite 101, West Caldwell, New Jersey 07006) to provide occupational therapy to out-of-district students at the Children’s Institute at the rate of $90/hr. and $67.50 per 45 minute sessions and per 30 minute sessions (not to exceed $6,000) for the 2015-2016 school year, in accordance with the information in the hands of each Board member (Acct. #11-000-216-320-01-19).
P-19. The Superintendent recommends, the committee concurs and I so move that the Board approve New Jersey Specialized Child Study Team (Katzenbach School for the Deaf) to provide Partial Evaluations Package at the rate of $1,350, Individual Evaluations at the rate of $400 per evaluation for the 2015-2016 school year (not to exceed $4,500), in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-20. The Superintendent recommends, the committee concurs and I so move that the Board approve The Cerebral Palsy of North Jersey to provide Augmentative and Alternative Communication Evaluations at the rate of $750 and $1,200 per evaluation, and to provide training at $150/hr. (minimum of two (2) hours per session) for the 2015-2016 school year (not to exceed $5,000), in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-21. The Superintendent recommends, the committee concurs and I so move that the Board approve Jewish Vocational Services to provide Vocational Evaluations at the rate of $875 per evaluation and Community Based Assessment at the rate of $60/hr. for the 2015-2016 school year (not to exceed $3,400), in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-22. The Superintendent recommends, the committee concurs and I so move that the Board approve The Cerebral Palsy of North Jersey for training on IPAD applications and use to meet student needs, as identified per the IEP, at the rate of $150/hr. (not to exceed two (2) hours) for the 2014-2015 school year, in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-02-19).
P-23. The Superintendent recommends, the committee concurs and I so move that the Board approve Assistive Tek to provide (a) Assistive Technology Evaluations with report at the rate of $1,000 per evaluation (not to exceed $3,000) (Acct. #11-000-219-320-01-19) and (b) Assistive Technology Training at a full-day rate of $1,200 and half-day rate of $600 (not to exceed $1,200) (Acct. #11-00-219-320-02-19), in accordance with the information in the hands of each Board member.
P-24. The Superintendent recommends, the committee concurs and I so move that the Board approve Cross County Clinical and Educational Services, P.O. Box 150, Ringwood, New Jersey 07456, for Child Study Team Evaluations in a foreign language at the rate of $825 per evaluation for the school year 2015-2016 (not to exceed $5,000), in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-25. The Superintendent recommends, the committee concurs and I so move that the Board approve Dr. Isabel Detrizio Carotenuto, 24 Heritage Drive, East Hanover, New Jersey 07936, to provide Neurodevelopmental Evaluations at the rate of $385 per evaluation (not to exceed $8,000) for the 2015-2016 school year, in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-26. The Superintendent recommends, the committee concurs and I so move that the Board approve The Family Center at Montclair, 155 Pompton Avenue, Suite 106, Verona, New Jersey 07044) to provide Psychiatric Evaluations at $450 and Mental Health Screenings at $100 (not to exceed $10,000) for 2015-2016 school year, in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-27. The Superintendent recommends, the committee concurs and I so move that the Board approve Dr. MarkFaber, 594 Valley Road, Upper Montclair, New Jersey 07043, to provide Psychiatric Evaluations at the rate of $550 per evaluation for the 2015-2016 school year (not to exceed $5,000), in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-28. The Superintendent recommends, the committee concurs and I so move that the Board approve Dr. Abraham Morganoff, Pediatric Neurologist, 1020 Galloping Hill Road, Union, New Jersey 07083, to provide neurological evaluations at the rate of $450 per evaluation (not to exceed $5,400) for the 2015-2016 school year, in accordance with the information in the hands of each Board member (Acct. #11-000-219-320-01-19).
P-29. The Superintendent recommends, the committee concurs and I so move that the position of Substitute Procurement Coordinator be abolished, effective July 1, 2015, in accordance with the informatin in the hands of each Board member.
P-30. The Superintendent recommends, the committee concurs and I so move the following hourly rates of pay for: (a) Suan Lam, substitute secretary, $20/hr. and (b) Barbara Marcianti, substitute secretary, $29/hr., in accordance with the informatini n the hands of each Board member.
